How Do I Make An Appointment?
Call us at (502) 806-3376 to make an appointment. We will ask that you describe the reason for your visit so we can schedule you with the appropriate dermatologist and for the adequate amount of time.
We are happy to schedule you with a provider of your choice. Please be prepared to provide your demographics (phone number, addresses, etc.) and current insurance information. Please note, we are unable to schedule and secure your appointment without this information.
How Do I Refill My Prescription?
Call your pharmacist to refill a prescription. The pharmacist will then call our office for approval.
Prescriptions can be approved only during office hours. Please allow 24-48 hours to approve a refill. If your refill request is denied you will need to contact the doctor.

What If I Cannot Make My Appointment?
If you are unable to make your appointment, please call (502) 806-3376 as soon as possible to cancel and reschedule your appointment.
What Do I Need To Bring For My Appointment?
For your appointment, you will need to bring your picture ID, insurance card, and insurance co-payment.
